Compound: Each tablet contains:

* Perindopril arginine 10 mg Amlodipine 10 mg

* Perindopril arginine 10 mg Amlodipine 5 mg

* Perindopril arginine 5 mg Amlodipine 10 mg

Properties: Combined antihypertensive and antianginal agent, contains perindopril arginine (ACE inhibitor) and amlodipine (calcium channel blocker).

Amlodipine is a slow calcium channel blocker, a derivative of dihydropyridine. Amlodipine inhibits the transmembrane transition of calcium ions into cardiomyocytes and smooth muscle cells of the vascular wall.

Perindopril is an inhibitor of the enzyme that converts angiotensin I to angiotensin II (ACE inhibitor). ACE, or kininase II, is an exopeptidase that both converts angiotensin I to the vasoconstrictor angiotensin II and degrades the vasodilating bradykinin to an inactive heptapeptide.

Indications:

Arterial hypertension and / or coronary artery disease (stable angina pectoris) in patients who require therapy with perindopril and amlodipine.

Dosage and administration:

The optimal dosing regimen is determined by the doctor. Compliance of the dosage form of a particular drug with indications for use and dosing regimen should be strictly observed.

Contraindications:

Amlodipine

— Severe arterial hypotension (systolic blood pressure less than 90 mm Hg);

— shock (including cardiogenic);

— obstruction of the outflow tract of the left ventricle);

— hemodynamically unstable heart failure after acute myocardial infarction;

— age up to 18 years (efficacy and safety have not been established);

— Hypersensitivity to amlodipine and other dihydropyridine derivatives.

Perindopril arginine:

— Angioedema (Quincke’s edema) in history (including against the background of taking other ACE inhibitors);

— hereditary/idiopathic angioedema;

— simultaneous use with aliskiren-containing drugs in patients with diabetes mellitus or impaired renal function (GFR <60 ml / min / 1.73 m2);

— pregnancy;

— age up to 18 years (efficacy and safety have not been established);

— hypersensitivity to perindopril or other ACE inhibitors.

Precautionary measures:

Use with caution in the following conditions:

Renal artery stenosis (including bilateral), the only functioning kidney, liver failure, renal failure, systemic connective tissue diseases (including systemic lupus erythematosus, scleroderma), immunosuppressant therapy, allopurinol, procainamide (risk of developing neutropenia, agranulocytosis), reduced BCC (diuretic intake, salt-free diet, vomiting, diarrhea), aggravated allergic history or history of angioedema, atherosclerosis, cerebrovascular diseases, renovascular hypertension, diabetes mellitus, primary hyperaldosteronism, chronic heart failure, combined use of dantrolene, estramustine, potassium-sparing diuretics, potassium preparations, potassium-containing dietary salt substitutes and lithium preparations, clarithromycin, tacrolimus, cyclosporine, CYP3A4 inhibitors or inducers, hyperkalemia, surgery/general anesthesia, desensitization therapy, LDL apheresis, aortic stenosis / mitral stenosis / hypertrophic obstructive cardiomyopathy, use in patients of the black race, CHF of non-ischemic etiology III-IV functional class according to the NYHA classification, old age.

Side effects:

From the nervous system: often — drowsiness (especially at the beginning of treatment), dizziness (especially at the beginning of treatment), headache, paresthesia.

From the senses: often — visual disturbances (including diplopia), tinnitus.

From the side of the cardiovascular system: often — a feeling of palpitations, flushes of blood to the skin of the face, a pronounced decrease in blood pressure.

From the respiratory system: often — shortness of breath, cough.
